If an adult volunteer will be serving in a role that interacts with campers, the volunteer may be subject to a background check. If that is the case, we will send you a link to complete that online.

ADULT VOLUNTEER AND RECREATIONAL ACTIVITY RELEASE OF LIABILITY, WAIVER OF CLAIMS, EXPRESS ASSUMPTION OF RISK AND INDEMNITY AGREEMENT

To volunteer at Round Lake Christian Assembly-Round Lake Christian Camp, a copy of this waiver must be completed by you. Please read the waiver in full and be certain that you understand the implications of signing.

Express Waiver of Risk(s) Associated with Recreational Activities and Volunteering:

I hereby affirm and acknowledge that I fully understand the hazards and risks associated with volunteering to assist in the many outdoor and other recreational activities in which I may engage or participate at Round Lake Christian Assembly-Round Lake Christian Camp. The inherent risks and hazards include but are not limited to:

1) Injuries sustained from any and all outdoor activities, such as running, jumping, hiking, swimming, biking, climbing, camping, cooking, engaging in sporting events such as basketball, volleyball and more.
2) Injuries sustained from objects that are either natural or man-made, such as rocks, cliffs, trees and campfires, or from misjudging trails or other terrain that induces slipping, falling, colliding or otherwise.
3) Injuries and illnesses from swimming, diving, scuba diving, cliff jumping, impacting the water, and/or water entering bodily orifices.
4) Injuries from hypothermia, heat stroke, dehydration, etc. from exposure to the elements, such as rain, cold, excessive heat or the weather in general.
5) Injuries or illnesses sustained from either plants or animals, such as poison ivy, poison oak, poison sumac, aggressive or biting pets, service animals, wildlife, or exposure to any plants or animals present within the camp in general.
6) Accidents, injuries or illnesses occurring in remote locations where no immediate medical attention is available.
7) If any member of your event or group is sick or may have been exposed to anyone with COVID-19 and attends an event or camp held at Round Lake, Round Lake is not responsible for the consequences thereof and waives all liability from the same.

I understand that the description of these risks is in no way complete and that all such dangers, both anticipated and unanticipated, can lead to illness, injury, permanent disability, drowning and death.

Release of Liability, Waiver of Claims and Indemnity Agreement:

In making it possible to volunteer and participate in many of the above-described activities and more, I both agree and acknowledge that:

1) As a committed Christian I will conduct myself in a reasonable and appropriate manner with campers (as well as others) at all times so as to glorify the name of Christ and to further the mission of Round Lake Christian Camp. I will report to the dean of the activity or the camp coordinator any inappropriate action toward any camper (as well as others) that I observe or reasonably believe to have taken place or that may likely take place.
2) The information contained in my application is correct to the best of my knowledge. I hereby authorize Round Lake Christian Camp and its designated agents and representatives to conduct a comprehensive review of my background causing a consumer report and/or an investigative consumer report to be generated for employment and/or volunteer purposes.
3) I understand that the scope of the consumer report/ investigative consumer report may include, but is not limited to the following areas: verification of social security number; current and previous residences; employment history, employment credit history, educational background, character references; drug testing, civil and criminal history records from any criminal justice agency in any or all federal, state, county jurisdictions; driving records, birth records, and any other public records. I understand that I may be required to submit to a criminal record check with the Bureau of Criminal Investigation and Identification at a later time.
4) I further authorize any individual, company, firm, corporation, or public agency (including the Social Security Administration and law enforcement agencies) to divulge any and all information, verbal or written, pertaining to me, to Round Lake Christian Camp or its agents. I further authorize the complete release of any records or data pertaining to me which the individual, company, firm, corporation, or public agency may have, to include information or data received from other sources.
5) I understand that Round Lake Christian Camp and its designated agents and representatives will take reasonable precautions to maintain all information received from this authorization in a confidential manner in order to protect the applicant’s personal information, including, but not limited to, addresses, social security numbers, and dates of birth, to the extent authorized by law
6) I will observe the following Code of Conduct as well as all rules and regulations of the camp as they may change from time to time while serving as a volunteer at Round Lake Christian Assembly and acknowledge and agree that I may be discharged from my responsibilities by the dean or camp coordinator for any reason or for no reason whatsoever: The Dress Code listed in the The Summer Camp Book applies to faculty; Faculty members shall be in the dorms no later than 12:30 a.m. and representatives shall remain therein until the next day’s program begins; Kitchen helpers no later than 11:30 p.m.; During the camp session and to the extent otherwise lawful and allowed, faculty are not permitted to date campers or one another; Waterfront rules apply to everyone; All baptisms at camp shall be under the supervision and instruction of the Dean.
7) Primary insurance coverage is the responsibility of the camper's insurance plan and faculty member's insurance plan. All hospital and doctor forms shall be completed using patient's name, address, and personal insurance.
8) This volunteer service, including engaging in recreational activities, carries with it the potential for serious injury, death and/or property damage, and I certify as to my physical fitness to participate and declare that I have not been advised otherwise by a qualified medical professional.
9) I will at all times be aware of the surroundings during the volunteer service and agree that if I consider anything related to this activity to be unsafe, will immediately advise the activity officials of such, and if necessary, will leave the area or refuse to participate further in the volunteer service.
10) I hereby release and hold harmless with respect to any and all illness, injury, disability, death, loss or damage to person or property—whether caused by negligence or otherwise—the following named persons or entities, herein referred to as Releasees: Round Lake Christian Assembly, Round Lake Christian Camp, and any and all other camp programs of Round Lake Christian Assembly.
11) I hereby release the Releasees, their officers, directors, managers, employees, representatives, agents, other volunteers, and other vessels, including others who give recommendations, directions, or instructions as part of this volunteer service, from any and all liability and responsibility and for any claims that I or my estate, heirs, survivors, executors, or assigns may have for personal injury, property damage, or wrongful death arising from the above activities whether caused by active or passive negligence of the Releasees or otherwise. By executing this document, I agree to hold the Releasees harmless and indemnify them in conjunction with regard to any illness, injury, disability, death, or loss or damage to me that can ensue from engagement in the full body of activities and volunteering as stated above and otherwise.
12) I authorize any medical treatment, including the administration of anesthesia, deemed advisable by any licensed physician to relieve any injuries received or illness contracted by me as a participant in this volunteer service. I hereby agree to pay all costs of any medical treatment or emergency transportation.
13) I authorize and consent to the Camp, its sponsors, and any news media, and their successors and assigns and those acting under their authority, to take, publish, use in any media, and copyright photographs, videotape or other and audio or visual media, including broadcast in any media, of me and agree that such may be used for any lawful purpose without further compensation or approval.
14) By entering into this agreement, I am not relying on any oral or written representation or statements made by the Releasees other than that set forth in this agreement.

This release shall be binding to the fullest extent permitted by law. If any provision of this release is found to be unenforceable, the remaining terms shall still be enforceable.
